Constant changes without investigating/confirming whether previously implemented improvement have started to work. Pure communication across functions in R&D. An attitude towards new eployees that everybody needs to gain his/her own experience without sharing information how a particular task can be done effciently; as there are no procedures understood as workflow for required activities to be completed in order to meet any milestone it makes a work of the team unefficient.
