Insufficient infrastructure tools, constantly changing processes, lip service ONLY in terms of work/life balance, huge pressure to "meet the numbers" - i.e. you hear words like "blood commit" - it is ALL about the number, middle management tends to shoot first based on hearsay (as opposed to getting all sides of a story) and ask questions later.
