Change the middle level of management employees that you have, and start to encourage fair play in promoting deserving candidates, people who have not contributed at all in the project they should never be promoted, what I mean is that there should be some amount of justification. Also, I feel that the appraisals should be done both ways, i.e. the team members should be allowed to rate there managers, this way the middle management people will become answerable.
