Unrealistic plans, unrealistic time lines, terrible leadership, no adherence to WTD (many work 60 hours plus), no concern for employee health, blame culture generated from board level down whilst taking no ownership of senior involvement or lack off, HR trying to look after employees and company alike but bullied into company outcome/bias, lying to customers about capability and delivery, many leaders at all levels are over promoted beyond capability, hire and fire mentality driven from the top.
