Most of middle management is over their head due to inexperience or being to afraid to tell the truth. Employees are regularly given new direction and left to make manager decisions due to lack of manager engagement. If management is in the office, they are always in meetings or attending some kind of bohemian management training. 20% of the employees are slackers who show up, are protected by their manager and bring down morale. Absolutely noone is held accountable for their actions no matter how large the mistake or how many times they make it. Nepotism and friendships play a large part in how employees are treated. Favoritisms is painfully obvious. Job responsibilities and expectations are not in line with job descriptions or postings. Tasks are expected to be performed with few written policies, little to no direct and many, many variations of each task.
