Some of the best people I've ever worked with though when things go wrong everyone has a solution but it becomes too many chiefs not enough Indians at times.

Communication is very compartmentalized and doesn't always flow to all involved stakeholders, this gets worse when pressure is high, or during very busy times. Archaic paperwork based systems and change processes allow a lot of details to fall through the cracks.
