Some middle managers are afraid of losing their jobs. Therefore they do not help their workers grow. They do not give the correct information to their managers therefore the executives do not get the correct information to make better decision. No management training, therefore the managers do not get the best work out of their people. Sometimes there are promotions without experience. Sometimes there are promotions because of friendship. Therefore, there is a lack of respect for management and low morale. Employees, only work and do what they have to so they will not lose their jobs. That is why the company will never grow to what they want to be. The employees who do the day to day jobs don't go above and beyond.
