• Lack of Security and Safety: Staff were frequently left without adequate support in dangerous situations (including incidents involving weapons and overdoses). Leadership often remained absent during critical incidents. • High Turnover: In a short period, multiple experienced Program Coordinators, Case Managers and other key staff left the organization. This left gaps in continuity of care and created unstable work environments. • Leadership Disconnect: Decision-makers were often off-site, with little understanding of the realities frontline staff faced daily.
