Reactive leadership and culture, fire fighting mentality, resources constrained, reward and bonus system leads to silos not teamwork, and under qualified leaders who don't know the basics of their roles and lack experience to be effective. A lot of 'shoot from the hip' and leaders get ahead by badmouthing others including their equals.
