Null process documentation, each time you change position people assume you already know what to do, who your stakeholders are and how the process works. Bad leadership, poor management skills, no follow-up on projects once they are closed, no strategy with targets and goals so you don't even know what you are being evaluated on, or if you are investing your effort in the right project (only some people got to participate in full impact projects and the rest were BAU and without any interest from directives)
