Lack of communication between programs, lack of desire to listen to and implement new ideas, lack of true leadership and ability to function as a whole organization. Anyone who pushes back on the mentality of "this is how we do it here" will bang their head against a wall until they burn out and leave in less than a year. I have seen it happen time and time again.
